Handling of Personal Information
This is an outline of the particular provisions SHIBUYA SCRAMBLE SQUARE Co., Ltd. (hereinafter, the “Company”) has put in place for the handling of personal information collected from the users of SHIBUYA QWS (hereinafter, the “Facility”).

2. Personal Information Usage Purposes
From the perspective of the management and operation of the Facility and of market surveys and other marketing, the Company uses personal information (except for specific personal information) collected from the users of the Facility for the following purposes.
(1) For entering into and fulfilling contracts including the provision of products/services, and performing post-contract management and after-sales services for products/services provided.
(2) For contact necessary for providing products/services (including when requesting that a shipping company send a product, etc.).
(3) For providing information regarding products/services and other information regarding the Company’s business activities.
(4) For investigating and developing new products and services, as well as software, systems, equipment, and machinery for providing these.
(5) For invoicing/collecting fares and fees regarding products and services, as well as credit protection (including when requesting that a credit company process payment by a credit card, etc.).
(6) For managing members, instructors, etc. at the facilities the Company operates, managing event participants, and providing various services.
(7) For market surveys and other surveys and research.
(8) For business analysis.
(9) For competitions, campaigns, etc.
(10) For sending invitations for or information about events the Company hosts, co-hosts, sponsors, supports, or cooperates in.
(11) For sending mail magazines and direct mail associated with the Company’s business.
(12) For entering into and fulfilling contracts with business partners, and performing post-contract management.
(13) For activities regarding CSR (corporate social responsibility).
(14) For receiving and responding to inquiries and opinions from customers.
(15) For ensuring customer and employee security.
(16) For managing customers’ lost articles.
(17) For managing facility, equipment, and machinery maintenance and usage states thereof.
(18) For performing tasks associated with other business handled by the Company.
3. Provision of Personal Information to Third Parties
In addition to cases pursuant to the provisions of law, the Company may, within the scope necessary for achieving the usage purposes stated in 2. Personal Information Usage Purposes, provide the personal information of Facility users to third parties such as the Company’s group companies, business partners operating the Facility together with the Company (examples: Loftwork Inc., Theatre Workshop Inc., and Transit General Office Inc.), etc.
The information provided is limited to name, address, telephone number, and the minimum necessary items. The information is provided to third parties in writing, or by post, telephone, fax, email, an electronic medium, or the like, and sufficient care is taken to ensure safety management in the transfer (in the case of email, an electronic medium, etc., when the personal information is provided as digital data, it is encrypted before being transferred).
Note that provision to third parties will be halted if there is a request to that effect from the user himself/herself. In such a case, contact the personal information counters listed in 4.
4. Procedures Regarding Requests for Disclosure, Etc.
With regard to personal information which the Company possesses and has rights to all of (1) usage purpose notification, (2) disclosure, (3) revision, addition, or deletion, (4) halt of usage or elimination, and (5) halt of provision to third parties (hereinafter, (1) to (5) are collectively referred to as “disclosure, etc.”), if the person to whom the personal information pertains or a representative thereof desires disclosure, etc., a request may be made in accordance with the following procedure.

(2) Required Documents
Fill in the required fields in the Company-specified disclosure, etc. request form, and mail it in together with verification documentation.

A. Request from Actual Person
As an identity verification document, one copy of any of the following.
Driver’s license, passport, Individual Number Card (front side only), health insurance card
B. Request from Representative
Both the following document verifying the power of representation, and an identity verification document (the same as the identity verification document listed in “A. Request from Actual Person”) are required.
a. Legal Representative
A document verifying legal power of representation, such as a copy of the family register showing the relation between the actual person and the representative or an adult guardian registration certificate (only documents issued within one month will be accepted).
b. Voluntary Representative
Company-specified authorization letter
(3) Handling fee
When requesting a usage purpose notification or disclosure, enclose a 1,000 yen postage stamp as the handling fee.
(4) Other
1) Method for Requesting Disclosure, Etc.
– No requests using forms other than the Company-specified disclosure, etc. request form will be accepted.
– If there is a problem in the matters recorded on the disclosure, etc. request form, the verification document, the handling fee, etc., the Applicant will be made aware of it and instructed to make a new request. Note that if no new request is made after 14 days from this guidance, the request will be considered withdrawn.
2) Response Method and Response Period
– The response will be sent by registered mail to the address listed on the identity verification document in the case of a request from the actual person, and to the address listed on the identity verification document of the representative in the case of a request from a representative.
– A response will be sent out 14 or more business days after the arrival of all necessary documentation (including the handling fee required for handling) to the Company.
